CAQH setup for independent physical therapy practices
Starting a PT practice or adding a therapist? William Avery helps prepare your CAQH profile, organize supporting documents, and turn missing information into a clear action list.
Our CAQH setup service for physical therapists supports the administrative work while the provider retains control of their information, required approvals, signatures, and attestation.
Discuss CAQH SetupFree 15-minute fit call. Bring your state, provider count, and target payers. No patient records or personal ID numbers needed.
Understand the role of the profile
The CAQH Provider Data Portal lets clinicians maintain professional and practice information and share it with participating organizations they authorize. CAQH now uses the DataSpring, powered by CAQH, brand; its clinician guidance says existing portal access and profiles remain unchanged.
A complete profile helps supply information for credentialing. It does not establish a payer contract, network participation, an effective date, or permission to bill as an in-network provider. Confirm the next required step with each payer.
Prepare before entering data
We help collect and organize the information relevant to your therapist and practice. Requirements depend on provider type, practice state, and the profile. We check the requested fields and documents rather than treating every clinician’s checklist as identical.
How we support setup
Identify an existing profile or the need for registration, the providers and locations in scope, and the practice contact. Agree on an authorized access and document-exchange process before work begins.
Organize records and help prepare the applicable fields. Where appropriate, coordinate CAQH practice-manager tools for shared information. The provider reviews information available for import into their profile.
Check profile prompts and document status. Maintain a list of missing records, inconsistent details, and questions for the provider, with an owner for each unresolved item.
Help the provider review the profile, complete required attestation, and check which participating organizations are authorized. Record the visible status and any remaining tasks; payer decisions stay with the payer.
What your practice receives
Your handoff shows what was prepared, what remains open, and who needs to act. Agree on the providers, locations, and reporting format in the setup scope.

The organizations reviewed for authorization, profile status at handoff, and questions requiring direct payer follow-up.
After initial setup
CAQH preparation is one part of a broader credentialing process. A payer may require its own application, additional records, or other steps. We identify that handoff so a completed profile is not mistaken for an approved enrollment.
Profiles also need updates when information changes and when re-attestation is required. At handoff, confirm the applicable portal reminders and who will respond. Ongoing maintenance, document renewal, and re-attestation support are separately scoped; initial setup does not automatically include continuing monitoring.
Common questions
Yes. Tell us whether the profile is unfinished, outdated, or missing documents. We review the starting point and agree which corrections and setup tasks are included before beginning.
No. We coordinate appropriate access and preparation tools with the practice. The provider retains their login and completes the reviews, approvals, signatures, and attestation required of them.
No. Profile completion and payer access are separate from a payer’s credentialing, contracting, and enrollment decisions. Confirm participation and the effective date with the payer before treating the profile as approval.
Continuing maintenance and re-attestation support require a separate scope. We agree who tracks portal notices, changed information, and expiring documents so the practice knows what remains its responsibility.
A clear starting point
Tell us your provider count, whether profiles already exist, and which payer requests are holding up the process. We’ll discuss a setup scope, missing records, and the provider reviews needed to move forward.
